• We have a strong collaborative team including Physicians, NPs, PAs, Lead Nurse Managers, Data Analyst, Pharmacist, Social Workers, and Psychiatrist to help improve patient healthcare outcomes.
• We have provided patient centered care coordination for over 8,800 MDPCP patients, among 45 providers within 15 partner practices in our CTO
• Our CTO supports its partners with patient specific resources to enhance provider care.
• We have the ability to access data to help you identify patient’s HCC risk adjustments to help improve patient care.
• We have expertise in Health IT, billing, and coding to help maximize physician revenue.
• We provide practice specific on-boarding, care management and data analytic tools to help your practice thrive.
